How Cortisol Is Measured


Cortisol is measured directly from a blood sample using an automated immunoassay. A standard blood cortisol test generally measures total cortisol, which includes cortisol bound to circulating proteins and the smaller unbound fraction.


Because cortisol changes substantially throughout the day, collection timing is essential. A morning result should be compared with a morning reference range and, for longitudinal tracking, with samples collected at a similar time after waking.


A routine morning cortisol measurement can provide useful context, but it is not by itself a diagnostic test for Cushing syndrome or adrenal insufficiency. Cortisol follows a circadian rhythm and usually rises around awakening before declining throughout the day. Even when two samples are both collected in the morning, differences in the amount of time since waking can affect the comparison.

Sleep and shift work

Sleep duration, sleep disruption, irregular schedules, overnight work, and changes in wake time can alter cortisol timing. A morning clock time may represent a different biological point for someone who works overnight or recently changed sleep schedules.

Acute stress, illness, pain, and exercise

Infection, inflammation, pain, anxiety around collection, intense exercise, surgery, and other physiological stressors can temporarily raise cortisol. A temporary increase is often part of the body’s normal adaptive response.

Glucocorticoid medications

Oral, injected, inhaled, and topical glucocorticoids can affect cortisol results. Depending on the medication, dose, route, and timing, they may suppress the body’s own cortisol production, interfere with an immunoassay, or do both. Recent dose changes and discontinuation are important to consider.

Pregnancy, estrogen, and binding proteins

Pregnancy and oral estrogen can increase cortisol-binding globulin, which may raise total blood cortisol without an equivalent increase in unbound cortisol. Changes in albumin and other binding proteins can also affect total cortisol interpretation.

Fructosamine provides context about average blood sugar exposure. Triglycerides, HDL cholesterol, LDL cholesterol, ApoB, remnant cholesterol, and the triglycerides to HDL ratio help show whether cortisol changes are occurring alongside shifts in lipid and glucose metabolism. hsCRP can provide additional context when acute or chronic inflammation may be influencing cortisol.


TSH, Free T4, and Free T3 help show whether cortisol changes are occurring alongside altered thyroid signaling. Total testosterone, Free Testosterone, estradiol, progesterone, and SHBG add reproductive and endocrine context because illness, energy availability, medication use, and physiological stress can influence several hormone systems at the same time.


Albumin provides additional context about circulating protein status and hormone binding. Creatinine, eGFR, GGT, and ALP can help determine whether a cortisol change appears isolated or is part of a broader physiological shift involving kidney or liver markers.
